I hereby apply for enrollment of my child in the Thomas Heyward Academy, Inc., for the school year 2005/2006:

I.            Applicable to all families:     It is hereby agreed that I will pay tuition in the amount of three thousand one hundred and eighty  ($3,180.00) dollars per child per year for grades K-4 through 12.  (If a family has more than 2 children, the tuition will be $3,060.00 for each child after the second one.)  It is hereby agreed that should the tuition be paid in 12 monthly payments, that said payment of two hundred sixty-five ($265.00) dollars shall be made on or before the 15th day of each month, beginning in June, 2005.  Should my child enter after the summer months, I agree to either catch up the 3 summer months or pay the tuition in 9 monthly payments of three hundred eighty ($380.00) dollars for September and three hundred fifty ($350.00) dollars October through May.  I also understand that failure to pay tuition on time will result in my child/children being dismissed from the Academy.  It is also agreed that I will pay a registration fee of $30.00 dollars per child for students currently enrolled in the Academy or $50.00 dollars for new students or current students who do not re-enroll by January 14, 2005, a book rental fee of $150.00 per child, a $25.00 fee for each typing, 7-12 lab science, art course and computer course (including K4-6 computer) my child may take, a $15.00 supply fee for each student in grades K4-6, a $5.00 locker fee (Gr. 4-12), and a $25.00 athletic fee per athlete per sport.

 

II.            Applicable to families applying for new students:   It is agreed that in addition to the fees listed above, I will pay a testing fee of $30.00 per child (excluding K4) and will make a two hundred fifty ($250.00) dollar donation to the building fund per child.  The building fund is due in full before the student may be accepted to the Academy.  I understand that I must supply a current and correct  SC Immunization Form, a copy of a birth certificate, and a social security number for each child before school starts in August.

 

III.       I UNDERSTAND THERE WILL BE NO REFUND OF ANY MONIES.

 

IV.        All students must abide by the rules and regulations, including the new dress code, i.e. uniforms, listed in the THA Handbook.  This document, together with the Academy Handbook and all rules, regulations and conditions in both, constitute the entire agreement and contract between the Academy, the student and the parent(s).  Should any portion of this document or the Academy Handbook fail to survive or be enforceable for any reason, the remaining terms shall survive and be enforceable.  No oral modification of this document or the Academy Handbook shall be valid, except the same be in writing, and signed by the parties to this print instant. 

 

 V.        I understand there will not be an opening for my child if he/she fails their present grade and that grade reaches capacity for next year.

 

VI.        The following rights and prerogatives shall be vested in the Academy:

1.            To determine the grade and section to which the child shall be assigned.

2.                  To suspend or expel the child for any scholastic or disciplinary reason or cause, and in the event of such suspension or expulsion, the Academy shall be the sole judge of the sufficiency of such     reason or cause.

3.                  To administer reasonable corporal punishment.

4.            To require full payment of fees in the event of the withdrawal of the child from THA for any reason             once he/she has been accepted.

5.            To refuse application based on criteria set by the Academy.

6.            To reserve the right to search the students, and their lockers at any time, or cause or allow law                                    enforcement to do the same.

7.            If necessary, to require the parents to have the child tested by a psychiatrist or psychologist                                  approved by the Academy.

8.            The school also reserves the right to search any vehicle entering or parked on school grounds.
